The date of the form is printed on the bottom of the form. The most popular is HCFA 1500 (1990). File by Modem refers to writing to a file that can be electronically transmitted electronicallysuch as Medicare claims in UTAH, but more generally, any claims that can be output to a file and subsequently transmitted.

Print Test Pattern--will print the letter x in all printable locations on the form to allow alignment with the printed information with the printer (pin feed printers-paper load position; ink jet and laser printers-margins and borders). Use this function when initially setting up the forms printing using the directions in the printer section. Reset Date Filed--will ask you for a date, ans will go into the invoice screen and reset dates filed on the only the invoices for which ONLY FORMS printed only on that date plus ONLY FORMS selected per screens 2 and 3 above; Electronic claims filed on the selected date are not affected.

Print Refiles--the criteria for printing claims that have already been filed are twofold: (1) the lapsed days since the first filing (see company screen-Insurance refile time) must have been exceeded; i.e. if the refile time is 45 days, then the first filing must be at least or more than 45 days ago. Print Mailing Labels--almost a thing of the past. However, if you are not using windowed envelopes, after printing forms, address lables can be printed, and will only include the forms printed today (see company screens for selecting left-right-both windows for envelopes). Reset Date Refiled-- works like Reset Date Filed above
Special Medicaid Forms - Examples below -- Georgia and Kentucky
Although most states use the standard HCFA1500 form, some states have devised spcial forms (heaven forbid the complications). Below are special screens for Georgia and Kentucky Medicaid. When you enter the State in company screen 1, your system is configured to the specifics of that state -- usually Medicare and Medicaid --
